School nurse Clinic based Health Services

In accordance with the Texas Senate Bill 12, Lampasas ISD is providing this annual notice of the school nurse clinic health-related services available to students during the upcoming school year. As a parent or guardian, you have the right to Consent (opt-in) or to withold consent (opt-out) for health realted services provided at your child's school nurse clinic.

These services may include (but are not limited to):
 
  • Basic First Aid & Injury Evaluation: Providing immediate care for illnesses and non-life-threatening injuries. (i.e taking temperature, cleansing a scrape & applying a bandaid, assessing stomach ache, assessing headaches). 
  • Health Promotion & Education: Educating students and parents on various health topics, including disease prevention, healthy lifestyles, and managing chronic conditions.
  • Administration of prescription medications in accordance with law: Medication Administration form signed by MD and Parents Required in addition
  • Monitoring of chronic health conditions (e.g., asthma, diabetes): Assisting students with chronic conditions that includes managing their health, administering medications and developing Individualized Healthcare Plans (IHPs). 
  • Vision, hearing, and scoliosis screenings: Required screenings by the State of Texas. Hearing and Vision screening is for K, 1st, 3rd, 5th, 7th and all new students to Texas. Scoliosis: Girls will be screened two times, once at age 10 (or fall semester of grade 5) and again at age 12 (or fall semester of grade 7). Boys will be screened one time at age 13 or 14 (or fall semester of grade 8). Opt-out is still allowed when a notice is sent prior to the screening event. 
  • Under the influence assessment
  • Lice screening: We follow AAP guidelines and CDC guidelines; notices will be sent for confirmed cases of lice in a classroom. 
  • Acanthosis screening: Screening for 1st, 3rd, 5th and 7th graders Texas Risk Assessment for Type 2 Diabetes in Children
  • Support during illness or physical symptoms at school
  • Coordination of health services: Collaborating with physicians, families, and school staff to ensure comprehensive care, advocacy and communicate health information effectively. (Medical Release required to speak with Physicians) 
  • Heat illness prevention and injury support for student athletes
  • Emergency Preparedness Training: Stop the bleed and CPR training required to graduate, typically held in 7th-8th grade. 
  • Community Outreach (Medical & Dental Referrals): Connecting students and families with community healthcare providers and social services. Can include vision vouchers from VSP/ Lions Club. 
  • State Immunization Compliance: Ensuring Immunization Compliance on shot records and ensuring Affidavits are not expired to allow enrollment into school per Texas Law. 
  • Skilled Nursing Care & Procedures: G-tube, feeding assistance, or other skilled nursing procedures per IEP/IHP/Medical order. 
  • Puberty Education Course: 4th & 5th Grade, Consent and Notice will be sent for parents to opt out if they elect in the Spring. 
  • Mental Health Support and Referral Services: Provide first-aid care and crisis intervention, then partner with and transfer care to the campus counseling department for further support if warranted. 
  • Good Side Health and TCHATT Services (Additional Consent is required): Good Side Health and TCHATT are partner companies that we use for mental health or wellness services, additional consents are required and parent consent prior to scheduling either service is required in addition to this consent. 
 
If you choose to opt out of the Texas-required screenings (hearing, vision, and scoliosis), you must provide a signed note from a healthcare provider confirming that your student has been examined and passed the required screenings.
Puberty education, however, does not require medical documentation if a parent chooses to opt out; a written opt-out response is sufficient for compliance.
 
If you choose to consent (opt-in) to the health‑related services and your child becomes ill or sustains an injury during the school day, the campus nurse will be permitted to assess and provide care to your student in a timely manner and return them to learning, when applicable. 
 
If you choose to withhold consent (opt out) of health-related services and your child becomes ill or sustains an injury during the school day, the campus nurse will not be permitted to assess or provide care, except in cases of a life‑threatening emergency.
If the nurse is not permitted to provide care for your student:
  • For minor injuries (i.e. scraped knee), a parent/guardian will be contacted to determine if the student can return to class or must be picked up in order for parent/guardian to provide care
  • For suspected illness, a parent/guardian will be contacted, and the student must be picked up in a timely manner. They may not return to school without a written statement from a licensed healthcare provider confirming that the student is not ill or contagious.
 

Action Required

 
To comply with legislative requirements, a parent or guardian must complete the Skyward Access form titled “25-26 Consent to Health Services” to either opt in or opt out of clinic-based health-related services for their child. Completing this form ensures we provide only the services you consent to.
 
You may update your preferences at any time by submitting written notice to your campus nurse or the Director of School Health.
 
If you choose to opt out of some, but not all, services, please complete the Opt-Out Consent Form provided below. Be sure to check the boxes next to any services you do NOT want your student to receive. This form must be returned to either the Campus Nurse or the Director of School Health.
