The Student Transfer Application is available from any campus office, the Human Resource Department, or you may click the link below to print the form.
- Complete the application for your student(s)
- Submit application, along with any campus-required documents, to the campus you are requesting. Please contact the campus to determine if any supporting documents are required at the time of application submission.
- Parent/Legal Guardian will be notified by mail of decision
All applications received by May 15 will be reviewed and parent/legal guardian will receive notification of campus assignment by June 5.
Applications received after the May 15 deadline will be reviewed following summer registration and notifications will be sent in early August.
