Ath. Training/Physical Info/Forms

    The UIL has made some exceptions for physicals for the 2020-2021 school year only.

    In the past new physicals have been required as 7th/9th/11th grade but there is an exception for 2020-2021.

     

    Every student-athlete / member of marching band must complete a medical history form (this has always been done annually even on years when a physical wasn't required).

     If there is a "yes" answer to any part of questions 1-6 then a new physical or written clearance is needed prior to participation.

     

    Only incoming 7th graders and those new to athletics or new to marching band are required to have a new physcial

    Here are some FAQ's that may help:

     

    My child participated as a 7th grader and will be an 8th grader, what do they need?     A completed and signed medical history form.  If questions 1-6 are a "NO" then nothing further is required.  If there is a "YES" to a part of questions 1-6 then a new physical or written clearance is needed.

     

    My child is going to be a freshman but participated as an 8th grader, do they need a new physical?  No, not for the 2020-2021 school year.  They do need a completed and signed medical history form.  If questions 1-6 are a "NO" then nothing further is required.  If there is a "YES" to a part of questions 1-6 then a new physical or written clearance is needed.

     

    My child wants to participate in sports and/or marching band but has never participated before, what do they need?   A completed physical by a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, matches or LISD sponsored camps/workouts.

     

    My child participated as a sophomore, what do they need for their junior year?  A completed and signed medical history form.  If questions 1-6 are a "NO" then nothing further is required.  If there is a "YES" to a part of questions 1-6 then a new physical or written clearance is needed.

     

    My child participated in athletics the previous year but had surgery since their last physical, what is required?  Written clearance from their surgeon and a completed and signed medical history.

     

    My child played sports as a 7th and 8th grader but did not play as a freshman or sophomore and now wants to try out as a junior, what do they need?   A completed physical by a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, matches or LISD sponsored camps/workouts.

     

    To clarify, if my child participated last year and has a clean medical history (no yes answers on questions 1-6) then they don't have to have a new physcial?   Correct, only those new to sports or marching band need a physical.

     

     We are new to LISD, can I bring my child's physical from their previous school?   Yes, provide their most recent physical and complete a new medical history form.

     

    My chlid only wants to try out for tennis, do they need a physical?  Yes, they need a physical on file.  If they participated in past years then a complete and signed medical history is needed.  If they are new to trying out for sports, no matter the grade level or the sport, they must have had a physical less than two years old.

     

     


    Annual Forms for Athletics:
     

     There is a new box on the medcial history concerning electrocardiograms.  An ECG is not required, but if you check the new box that alerts us to expect your student to have an ECG.  If you child has not had an ECG and you don't plan on having an ECG the please do not check the box.
